I, the undersigned parent or legal guardian of the above-named child, acknowledge that participation in soccer and related activities at Eagles FC Academy involves inherent risks, including but not limited to physical injury, property damage, and other potential hazards. I voluntarily assume these risks on behalf of my child.
In consideration for allowing my child to participate in the activities organized by Eagles FC Academy, I hereby release, waive, discharge, and covenant not to sue Eagles FC Academy, its officers, agents, employees, and volunteers (collectively, "Releasees") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, whether caused by the negligence of the Releasees or otherwise, while participating in the activities or while in, on, or upon the premises where the activities are being conducted.
Medical Treatment Consent:
I hereby authorize the Academy and its representatives to obtain emergency medical treatment for my child in the event of injury or illness. I understand that all expenses related to such treatment, including transportation, are my responsibility.
Indemnification:
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs and attorney's fees, that may arise due to my child's participation in Academy activities, whether caused by the negligence of Releasees or otherwise.
Photography/Videography Consent:
I consent to the use of photographs or video recordings of my child participating in Academy activities for promotional and educational purposes. I understand that these images may be used in print, online, and other media.
Acknowledgment of Understanding:
I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
